Fonteum is a US healthcare provenance registry that publishes signed, chain-of-custody-attested research and data pages on Medicare, Medicaid, and federal regulator datasets, drawing from 44 federal source families across CMS, OIG, HRSA, AHRQ, and HHS.

Every published field carries the same 14-field provenance contract — source, source URL, dataset ID, snapshot date, methodology version, last-checked date, confidence tier, data availability, pipeline version, a persistent-identifier slot, license, and coverage-period start, end, and SLSA provenance link. Each record is sealed with a chain-of-custody signature (Ed25519, in-toto format) and is publicly reproducible from the retained source bytes.
